This is an unofficial homebrew magic item. It is up to the DM if they wish to allow the item to be used in a campaign.

The Fang of Yeenoghu is a powerful magical item that can only be obtained by killing and harvesting two items from the demon lord Yeenoghu — a canine tooth and the creature's amygdala. The tooth needs to be polish, cleaned, and magically prepared. The amygdala must be soaked in an alchemical mixture for a week where it hardens into a reddish stone. The stone is then polished with a magical residue, resulting in a dark red pearl. The tooth and the pearl are then combined during a ritual under a full moon. The final item is added to a chain or string, and worn as a necklace.

While wearing this necklace, you gain the following benefits:

  • You have advantage to Wisdom (Survival) checks when tracking creatures.
  • You have advantage on Wisdom (Perception) checks that rely on smell.
  • On the first turn of combat, you can move double your base movement speed.
  • When you reduce a creature to 0 hit points with a melee attack, you can use a bonus action to move up to half your speed and make another melee attack.
Note: The Fang of Yeenoghu is a powerful magic item, that should only be given to powerful characters. As such, this charm should only be awarded to a high level party that has defeated the powerful demon Yeenoghu.
