This appears to be a leather harness with a flattened 18-inch pocket on the back. The backpack can be worn and concealed under any kind of clothing, making it unnoticeable. When the pocket is opened, the interior looks like a regular backpack (just with a smaller opening). It can hold 1 cubic foot/30 pounds of gear. Note: Sharp objects placed into the pocket have a 2d20 chance of piercing the backpack. Once the backpack is pierced, its magic is destroyed, and it can only hold 10 pounds of very flat gear.
